The video opens with a close-up of a woman mid-procedure, a large pocket of skin open to reveal a fatty yellow mass inside. As the doctor pokes and prods inside, it's difficult to discern what's fat and what's lipoma. (A lipoma, as a refresher, is a "knot of fatty tissue located just below the skin," according to the Cleveland Clinic . They can form pretty much anywhere, but they're most commonly found in the neck, shoulder, and generally around the trunk of the body. )

In a new YouTube video posted on Monday afternoon (and an accompanying Instagram post ), Dr. Pimple Popper-aka, dermatologist Dr. Sandra Lee, MD-diligently removes a lipoma that has made an unwelcome return.

Turns out, we're not alone in our struggles to figure out what's what-even Dr. Lee seems to have some trouble at times: "The goal is to get out as much as you can of the lipoma, but not regular fat," she says, "and kind of the problem is you can't tell where it is and where it starts."
She also alludes to the fact that this isn't her patient's first experience with having a lipoma removed, either. "You can see when a lipoma partially comes out, you try to get it out," she says. "Sometimes if you can't, maybe it tunnels and creates a little scar tissue and closes down in one area, and it's hard to get those little pieces out."

A wrestling match of sorts ensues-shaking, squeezing, snipping, and more. It's a long struggle, but as always, Dr. Lee emerges victorious, and carefully stitches her patient back up.
